Dedicated team of Social Workers and Community Health Workers whose mission is to help community members affected by dementia and related conditions to stay in their homes as long as possible.
What We Do
Caregiver support, ranging from respite services to help navigating health care systems, for caregivers of adults with debilitating illness, including dementia and Alzheimer's. Services are focused on supporting community members in the Greater Twin Cities area.
