Who We Are
Elder Connection provides older adults with services that promote healthy aging. Elder Connections honors people’s unique lived experiences and takes place where people need it most – in their home and community. Elder Connections serves older adults who have low-wealth and live in Hennepin and Ramsey counties.
Sometimes the bridge to a healthier community is someone who understands the whole person. Our service specialists and volunteers help build connections to people and resources, fostering overall health and well-being.
What We Do
Creating opportunities for health and connections
Older adults’ health and well-being improve when they have varied opportunities to stay socially engaged and connected to their community. But right now, far too many elders are isolated because they lack places to go and the means to get to those places. Our service specialists provide access to community resources and social opportunities to older adults to help them remain healthy, connected, and safe-at-home in their community.
Our team strives to meet residents where they are, serving people in their homes and communities.
Every person is unique, which is why we never use a one-size-fits-all approach. Our service specialists respect the experiences and wisdom of the people they serve and keep their voice at the center of care. Services are tailored to the person’s individual needs and with respect to the level of care they prefer.
Services may include:
- Health care coordination
- Economic counseling
- Housing Stability services
- Family mediation
- Group transportation for necessities (i.e., groceries, prescriptions, household items, etc.)
- Mental Health support
